[News Boquete] Message for U.S. Citizens: CHIRIQUI Outreach

American Citizen Services (ACS) and the Federal Benefits Unit (FBU) of the Consular Section are planning a visit to David and Boquete, Chiriqui.  An ACS Officer, Foreign Service National, and FBU representative will plan to be in Boquete on Wednesday, May 14th and David on Thursday, May 15th, 2014, to provide consular services to American citizens in the area. 
 
We will be at the Boquete Library from 09:00 am to 1:00 pm, on Wednesday, May 14, 2014.
 
We will be at the Hotel Ciudad David from 9:00 a.m. to 1:00 p.m. on Thursday, May 15, 2014.  
 
ACS will assist with the following services:
 
1.       Applications for U.S. passports (These services will be processed in Panama City)
 
a.       Passport Renewal (adults and minors)
 
b.      Adding passport pages (must surrender passport, pages added in Panama City)
 
c.       Lost or Stolen passports
 
2.       Notarizations.
 
3.       Registrations/STEP program
 
4.       Abandonment of Lawful Permanent Resident Status (Bring form I-407 and Permanent Resident Card)
 
5.       Delivery of new passport (must request service and provide authorization letter)
 
6.       Voting registration (please print and complete the form at: www.fvap.gov)
 

 

FBU will provide the following services:

 
1.      Application for a replacement SS card
 
2.      Application for a first time SS card for children under age 12
 
3.      Application for SS benefits
 
4.      Enrollment in IDD Program
 
5.      Enrollment/cancellation of Medicare
 
6.      Update contact information for SS purposes (Form SSA-21)
 

 

Services will be provided on a first-come, first-served basis.

 
No visa services will be provided.
 

 

Also, the Embassy's Ambassador will host a Boquete town hall from 9:00 – 10:00 a.m. at the Boquete Library.

 

 

IMPORTANT NOTICE

 
1.      Please bring photocopies of all documents to be submitted.
 
2.      Please bring the exact cash amount to pay for the service.
 
3.      Please bring completed forms.  Please access the following link for information on fees and forms you need to download, complete, and bring:  http://panama.usembassy.gov/service.html
 
4.      Check the instructions below to use a courier (DHL/FEDEX/Fletes Chavale) to receive your new passport.
 

 

Courier Instructions:

 
You will need to go to your nearest DHL/FEDEX/Fletes Chavale office and send us your old passport with a prepaid envelope.  When we receive your package, we will cancel your old passport and send you the new one and the old one.
